And Inside Macintosh Vol. 1, which was the current Dialog Manager reference at this point, provided the example shown below. IIRC the convention to place the default button in the lower right corner happened in the '90s, post-System 7.

Mac keyboards prior to ADB (SE/II) didn't even have an Escape key. (There's a great HIG archive on GitHub; see https://github.com/gingerbeardman/apple-human-interface-guidelines/blob/main/1987%20Apple%20Human%20Interface%20Guidelines%20-%20The%20Apple%20Desktop%20Interface.pdf).
