Depending on his weight that could be quite a high dose of ivermectin (the normal dose is 0.2mg/kg) but that seems like an extreme reaction. I see four possibilities: He’s very light weight and sensitive to the drug He’s having an allergic reaction He actually did have a parasite infection and he’s having a Mazzotti reaction. The company that sold it didn’t mix the drugs properly before compounding them and overdosed the pills.

$500 bottle of ivermectin!?!?! Somehow, that’s the worst part of this to me – somebody got hugely ripped off. I regularly need to buy ivermectin for actual horses, and it’s $10-$15 for a horse-sized dose. And this dumb motherfucker spent $500 on a single bottle?
