@spli I think I managed to get through the entire form without providing an email address or similar: while I'm not too difficult to find, I think that might be one step more difficulty than ideal!

I'm on the Zulip if nothing else works and happy to provide an email address. For now I need to get to bed, but figured I should let you know there's been a hiccup. And, er, I really ought to remember to ask MS Office forms to send me a copy.
