n.b. "hurting someone else's feelings" is not harm.
bluntly, to make this textual, Doc Impossible is trans and is talking about transitioning.
some people avoid transitioning because they're afraid it will "harm" people close to them.
your choice to transition is about your own body; if someone else chooses to take offense, that is on them - and if they try to stop you from controlling your own body, that is a grievous harm -to- you.
do the fucking thing. do it scared, but do it. and by "do it" I mean take some cross-sex hormones.

if anything you'll de-stress any of the queer people in your life who have been holding back from what they perceive as the obvious 'right' way to treat you, and are studiously treating you according to the mask you're insisting on maintaining. because yes we can fucking tell when someone is holding back from the obvious. it gets incredibly fucking obvious in some cases. which reminds me, I should remember to pack some spare estrogen for the fucking -egg- going camping with my group this weekend, in case -this- time that individual wants to actually fucking go through with it.
