We punish people for hitting the big red button, but then wail because we got a fatal HAI, wrong-site, wrong patient, or Never Events just keep happening. "Not speaking up" was the cause of the KLM Flight 4805 disaster and why we changed cockpit communication across the entire industry. I have seen toxic organizations and that flowchart is kinda how they function, and how people get killed So again, if this was humor, sure np @clew@ecoevo.social @michcampbell@fediscience.org
