EARLY in my SA career, I rebooted a production database server because I thought I was logged in to the dev or T&I one (they all had the same hostname, just different subdomains and only the hostname showed up in the shell-prompt). Since that incident, I've always used different color windows for different environments. No "did the right thing in the wrong place" incidents since.
