I saw a post last week about form data collection that I can't find...... and I've been tasked to develop a form to capture volunteer data for a group I work with..... I hope I've got things right, I'm certain that it's orders of magnitude better than the Word document that preceeded it! ;) The Age 18+ choice triggers some conditional, additional collection later in the process.
