Under pressure (and a legal requirement), the Trump administration released FBI files on a woman who accused Trump and Epstein of raping her when she was a teenager (per NYT). Journalists going through the big release of files noticed an index describing the pages, but the actual pages were missing. The DOJ now claims that they were "mistakenly" marked as duplicates.
