I've played on almost everything in this list except the witch's hat and swinging gates, which I don't recall ever seeing before. Saw our fair share of injuries too.

When she was maybe 7 or 8, my sister fell from the top of metal monkeybars and landed with one of the bars right between her legs. Serious fractures there.
