I won't tire of telling colleagues to:
1: Always assume what your patient tells you is correct unless you have a strong suspicion they're in a severely altered mental state. In that case, try to figure out how to help them best based on outside clues.
2: Talk to people. They'll tell you what they need. If you have more information, present it in an understandable format to them, and let them be an active part in creating their treatment plan.
3: Learn CRM guidelines.

@rogue_cells@chaos.social Amen to that! On bad days I would like instant electric shocks delivered after someone says "that can't possibly hurt" or words to that effect.
