From Linux (maybe WSL too) you can run Qb45 and it's successor PDS in DOSEMU including in console mode. At that point it's convoluted enough that I'd consider seeing if FreeBasic suits you better, its multi platform, and has some level of QBasic compatibility. Since you're in it for the long term, I'd consider opening some tickets against QB64 about accessibility features, it's always worth asking even if stuff takes a long time.
