Admitedly that can still happen here with lone overworked admins on instances (if they fund the instance then it is their rules but mistakes are easily made when you are firefighting). Two different models of decentralisation (others will disagree re that term in relation to Bsky), each with their own pros and cons. I like to think it is more granular in the fediverse in terms of options.

Can we get a neutral third-party who can stand up ethics and operational standards that center and ground informed consent and observe our own reported best ideals in relation to agency autonomy and lived experience authority in the context of social engagement within a public commons free from the domination of vested interest, which is traditionally not at all social.
