First held in 1093, one team wears vermillion (Kamigamo-jinja) and the other black (Iwashimizu Hachimangū).
One horse starts a length ahead of the other. The winner is determined by whether the gap between the horses has shortened or not.

The 'rachi' (enclosure) gate was opened to admit the horses, indicating the next race was imminent. Often spectators were left waiting for what seemed an age as it took time to calm horses & riders before the race. Over time the phrase came to mean "squeezing blood from a stone".
