It never ceases to amaze me how Auto Insurance companies in Michigan are allowed to require "customers" to jump through hoops to decline their overpriced medical coverage. At first, they required a letter from a medical insurance, then they've added forms that are confusing (at best). If you miss the letter, or one of the forms, they immediately increase your premium until you comply.
