I think he was joking :)
But if you want to know it’s because of an anatomical variant / birth defect where the testis is not fixed to the adjacent membrane called the tunica vaginalis.

The “they want to be even” explanation is what I remeber being told by the urologist who untwisted me. I was, to be fair, young and in pain, so I don’t know that I remember clearly. (Also, it was around 25-30 years ago, so I get to be wrong about this.)
