I’ve watched enough airplane accident videos to know that they don’t say over. Also it can result in one side literally not hearing the other side at all

How does saying over result in not hearing the other? Also, you say "roger" to confirm you heard something. If the other side doesn't say roger, you repeat the transmission or say "sayback" to have them confirm what they heard. And if you missed a transmission or it wasn't clear, you say "say again your last."
