It's usually written as where they're from and where to. Italian-American would be an Italian who immigrated to America.
He could be on vacation in Canada I suppose.
But it's most likely more American exceptionalism from the meme creator who assumed it was invented there.

I read it as it's the immigrant's first time seeing pineapple added to pizza, not the first pineapple pizza ever. It looks like they already knew how to use it as a torture device in the picture.
