Incident Reality If incidents are increasing, it’s not a technical issue. It’s a governance gap. Systems don’t suddenly become unstable. They reflect: • Poor change control • Weak ownership • Lack of discipline Fixing incidents without fixing governance… It’s just resetting the clock.
