How do you mean, “question what genitals are all about”? He just didn’t know his dad used to have a vagina (or still has one, I did invent the part where he may have had bottom surgery). That doesn’t mean anything about whatever else he might know or not know. I think I’m confused about what you’re confused about? 😅
