It was added after Trump took office. There were articles about it. Apparently labeling doorways is helpful for dementia patients, and many people took this as a sign of dementia.

As someone who works with dementia patients daily, he very clearly has it both by his actions and those of everyone around him. I would be shocked if there’s anyone left paying attention that genuinely thinks he’s sane. But I am often shocked at what people believe.
