Fast food just shrinks portions and jacks up prices in response.

It does, indirectly. They are typically more calorie-dense, especially high in carbs. So the same volume of food gives you more energy to store as fat.

Calories in calories out, except, fast food has minimal micronutrients therefore the body craves micronutrients and asks for more food. You eat more fast food and the body is satisfied because it's been fed. But then it digests the food and goes "damn, I need more micronutrients, more food please!" so then you eat more. Suddenly you've put a lot of calories in and now you're unhealthy because you're deficient in a bunch of things and have eaten far more salt, sugar, and fat than you should. Now you're fat and getting healthy is even more difficult. So you eat some fast food to feel better about yourself...

Calories in calories out is one of those things that's technically true but useless. Fast food is designed to be morish and addictive and is very calorie dense as others have said. So technically 3000 calories is 3000 calories no matter where it comes from but if you're actually interested in your health and losing weight the specific food you eat is going to make a huge difference to your ability to actually succeed long term. But yeah, calories in calories out. That doesn't mean all food is the same.
